This is the format for Creativ.email on wordpress, hubspot had a similar format. you have to put first and last name, and its not simple enough for what i want.

I want something like this. where you just put your email in and hit subscribe
I tried deleting the other boxes but it would only let me delete the text, leaving the boxes.
