I feel like this is really simple, but I can’t find a solution to my problem.

I sell customised jewellery. Customers can click “Add to cart” on product page, but then I need a pop up where customer can:

1. answer 4 questions e.g thickness, preference, design etc
2. Upload 6 photos of their jewelery
3. Click “Continue shopping” or “Proceed to check out” once complete

Then, once customer is finished shopping, they can proceed to checkout, where their product purchase AND the completed form is visible. (Refer to screenshot for example)

Any idea how I can achieve this? Really appreciate any help!

I have been able to create this solution on Squarespace beleive it or not, but for other reasons, I cannot use squarespace…the solution needs to be on WordPress.
